How to Enroll
You can enroll your student online, in person at one of our 18 enrolling schools or at the district office. Roosevelt is an open-enrollment district. Families residing outside of our district boundaries may apply at any time during the year at any of our campuses. A.R.S. §15-803 requires school attendance for every child between the ages of 6 and 16.
When you register your student, please have the following documents:
- Student’s valid birth certificate
- Student's current immunization record
- Valid proof of residency or address
Kindergarten Eligibility
A child is eligible for admission to kindergarten if the child reaches the age of five before Sept. 1 of the current school year, per A.R.S. §15-851.

What if I do not have a birth certificate for my student?
To help trace the location of any child who is reported missing, A.R.S. § 15-828 requires parents and guardians enrolling children to provide one of the following:
- A Certified copy of the child’s birth certificate.
- A letter from the authorized representative of an agency having custody of the child certifying that the child has been placed in the custody of the agency as prescribed by law.
- Other reliable proof of the child’s identity and age, including the child’s baptismal certificate, an application for a Social Security number, or original school registration records and an affidavit explaining the inability to provide a copy of the birth certificate.

Can my student start school if they are missing immunizations or I don't have a copy of their records?
Children cannot start school without having current immunizations and complete records. Immunizations may be obtained from the child’s last school on request.

What is accepted as a valid proof of residency or address?
Parents or guardians enrolling a student must provide one of the following items:
- Certificate of tribal enrollment (506 Form) or other identification issued by a recognized Indian tribe that has an Arizona address.
- Most recent bank or credit card statement.
- Most recent documentation from state, tribal or federal government agency (Social Security Administration, Veteran’s Administration, Arizona Department of Economic Security).
- Most recent pay stub.
- Most recent property tax bill.
- Most recent W-2 wage statement.
- Most recent water, electric, gas, cable or phone bill using the service address.
- Real estate deed or mortgage documents.
- Temporary on-base billeting facility, for military families.
- Valid Arizona Address Confidentiality Program authorization card.
- Valid Arizona driver’s license, Arizona identification card or motor vehicle registration.
- Valid Residential lease or rental agreement, signed by both landlord & tenant, including Arizona Section 8 agreement (issued by City government).

Can my student start Kindergarten if they aren’t five before Sept. 1 of the current school year?
Your student would have to pass the Early Entrance Exam, given near the end of May. Please contact our Early Childhood Department at 602-243-2613, no later than Mid-May, to set up a time and date for the exam.

How do I know what school my student will attend?
School attendance is specific to school boundaries. Please consult this map for specific boundaries. If you need more help, please call 602-243-4832.

What is the dress code for Roosevelt students?
All RSD students must adhere to the district uniform dress code. Please review each school’s uniform options and contact your student's school with specific questions, including assistance in getting uniforms.

Will my student start the same day I enroll them?
You may be asked to have your child start the next day if it is at a time when it would be a disruption to the class to have your child start that same day if school is already in session.

What is the Home Language Survey and why must I fill one out?
All students must fill out the Home Language Survey, sometimes referred to as the PHLOTE or Primary Home Language Other Than English form, even if their home language is English, per Administrative Code R7-2-306(B)(1), (2)(a-c).
The responses to the Home Language Survey are used by the school to provide the most appropriate instructional programs and services for the student. The answers will determine if a student will take the Arizona English Language Learner Assessment (AZELLA).
We ask parents to respond to each of the three questions as accurately as possible.

Can I enroll a student if I take care of them but am not their legal guardian?
The person who has legal guardianship of that student must completed the enrollment paperwork. The paperwork can be filled out at home and returned later.

How do I enroll my student if I do don’t live within Roosevelt boundaries?
Please contact the school of your choice to fill out an Open Enrollment Form. Acceptance is based on available classroom space and, at the discretion of the School Administrator, good attendance and good behavior. You must provide transportation for your child.

What is the process to move schools if I would like my student to enroll at another school within the district?
You must complete an open enrollment form at the prospective school of choice and the administrator at that school must accept the application.
